Dr. Jasmine Flake-Warmoth

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

  • Dr. Jasmine Flake is a board-certified endodontist that was born in Los Altos, California. She grew up in a family of dentists and spent her summers working alongside her mother in her dental practice. This early exposure instilled in her a deep commitment to patient care, and she treats every individual as an extension of her own family, with personalized attention and compassion.
  • Dr. Flake’s academic journey began at UCLA, where she earned a Bachelor of Science in Biology with departmental honors, alongside a Bachelor of Arts in World Arts and Cultures. She went on to receive her Doctor of Dental Surgery (DDS) degree from the University of the Pacific, Arthur A. Dugoni School of Dentistry, and completed a residency in Advanced Education in General Dentistry (AEGD) at the V.A. Long Beach Healthcare System. She then practiced as a general dentist in both Northern and Southern California, gaining valuable experience in comprehensive care.
  • Driven by a desire to provide more specialized care in a field of dentistry focused on saving teeth and taking patients out of pain, Dr. Flake pursued advanced training in Endodontics at NYU College of Dentistry.
  • Her love for education and mentorship has led her to serve as a pre-clinical faculty member at the University of the Pacific and as an honors endodontics instructor at NYU.
  • Dr. Flake is also deeply involved in organized dentistry and currently holds leadership positions within the California Dental Association and the San Francisco Dental Society, where she continues to advocate for both patients and dental professionals.
  • In addition to her clinical work, Dr. Flake is committed to serving her community. She has volunteered with organizations such as CDA Cares and San Francisco’s Project Homeless Connect, and has participated in numerous outreach programs.
  • Outside of dentistry, Dr. Flake enjoys hiking, kayaking, and spending time at the beach with her husband Adam and their dog Camper. She loves to travel, play tennis, and go out for a fun dinner with friends as living in New York previously turned her into a bit of a foodie.

 

 

 

 

 

 

 

Dr. Anne Kim

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

  • Dr. Kim holds not just one, but two Doctor of Dental Surgery degrees from Yonsei University in South Korea and the University of California, Los Angeles (UCLA) in the United States. She holds multiple honors throughout her distinguished academic journey and has a total of 6+ years in the study of dentistry alone.
  • With many years of experience as a general dentist in South Korea and the Los Angeles area, Dr. Kim developed extensive expertise in comprehensive dental care. This solid foundation allowed her to gain a deep understanding of overall oral health and patient needs, enabling her to approach complex dental issues with a holistic perspective.
  • After 5+years as a general dentist and driven by a passion for endodontics, she pursued advanced training and completed her 2 year endodontic residency at the University of Missouri-Kansas City (UMKC). This specialized training equipped her with expertise in root canal therapy and other complex endodontic procedures. Her proficiency allows her to effectively alleviate pain and save teeth that might otherwise be lost, significantly improving her patients’ quality of life.
  • Dr. Kim is a Board Eligible candidate in the process of completion of the American Board of Endodontics. This is yet another reflection of her commitment to the highest standards of excellence in her field. As an active member of the American Association of Endodontists (AAE), she stays at the forefront of advancements in endodontic care. Her extensive experience as a general dentist, combined with her specialized endodontic skills, ensures that each patient receives comprehensive and compassionate care in a comfortable and trusting environment.
  • Beyond her clinical work, Dr. Kim is deeply committed to giving back through volunteer service. She has participated in overseas mission trips and Mission of Mercy clinics, providing much-needed dental care to underserved communities.
  • In addition to her volunteer efforts, Dr. Kim is an avid traveler who loves exploring different cultures. In her free time, she enjoys playing beach volleyball, hiking, and spending time with her friends and family. She is excited to call the Bay Area her new home.

  • As the daughter of Vietnamese refugees, Dr. Phan is conscious of the generosity, kindness and support of others that provided her the opportunity to be where she is today. Pro bono treatment provided to those in need, working with Easy Bay Stand Down for Veterans, CDA Cares, Give Vets a Smile, and Medical/Dental Mission trips to Mexico in the past are all part of Dr. Phan’s philosophy to give back.
  • Work Experience
    • Dr. Phan is most proud of her work experience.  This is what has shaped her clinical commitment, expertise, philosophy, and how she leads her team. Becoming a skilled clinician with compassion rather than being a doctor scientist with accolades was and still is her goal today.
    • She has performed 1000’s of endodontic procedures and surgeries for the sole intention to retain the natural dentition for over 20 years in diverse settings. Dr. Phan practiced as a general dentist in multiple practices in Chicago and the Chicagoland Suburbs performing only root canal therapy.  In California,  she worked as an Endodontist in multiple general practices in Walnut Creek, San Jose, Pleasanton and for a large dental clinic in West Sacramento. At the same time, she was an Endodontic Associate in Pinole with a major emphasis on restorative endodontics and microsurgery.

  • Outside of work, Dr. Phan has 2 teen-aged daughters: one at Carondelet High and one at San Ramon Valley High. She enjoys long walks with her dog and spending time with her husband, who is also an Endodontist.
